Federal Tax Exemption Forms

For FFA chapters who do not use their school district EIN and wish to utilize National FFA's IRS Tax Number, please read the following documents carefully and complete the appropriate forms. The request form must be on file with NFFA and the 990 forms must be filed annually with the federal government. This is only for federal exemption, state specific forms must be completed as well. National FFA Affiliate Program

Every agricultural education program can ensure that its students will be able to participate in FFA activities. Find out more about the National FFA Affiliate Program.

Award Medal and Certificate Requests

Award medals and certificates are available to FFA advisors for chapter award winners, free of charge.

To request medals and certificates fill out the 2013-2014 Award Medal Request Form and follow the directions found in the form.

CDE, Agriscience and National Chapter Certification Content

CDE, Agriscience and National Chapter declarations and certifications are all completed online. State staff and qualified advisors can click on the link below to access their state and chapter’s certification page. You must be logged in to AgCN to access certification link. All team/individual changes will be made on the online certification site.
